Step 2: From the left-hand navigation menu, go to 'My Accounts' > 'Club Account', and select your club.
Step 3: Select 'Operations' > 'Active Programs' under the Travel Billing section.
Step 4: Under Active Travel Billing Programs, Select the Program Name or Add Program in the upper right hand corner of the screen.
Note: If you choose to Add Program A fly-out will populate, Travel Billing > complete information > click 'save'.
Step 5: Next select 'Custom Questions'.
Step 6: Select 'Player Questions'. Complete the following questions or click 'Add Questions' in the upper right hand corner of the screen.
Note: If you choose to add questions A fly-out will populate, Add Questions > select 'Pre-Set Questions' or 'Custom Questions' complete information > click 'save'.
